Hip problems

The acetabulum is the hip socket, where the femur (leg bone) and pelvis meet. Acetabular refers to the hip joint.

The hip joint can be subject to various traumatic injuries and medical conditions.

As far as traumatic injuries go, the most common is an acetabular fracture, often sustained during high-velocity impacts (such as road traffic collisions). Typically, the top of the femur is pushed upwards, crunching into the pelvis.

With regards to medical conditions, the acetabular can be affected by arthritis in the hips, which can significantly impede mobility. Acetabular dysplasia is another chronic condition, and refers to a shallow or misshapen hip socket that is present from birth.

Other problems that can affect the acetabular joint include cysts, labral tears, notches, necrosis, osteophytes, osteoporosis, sclerosis and tumours.

Acetabular revision surgery

If the patient's condition is very bad, surgery may be needed by way of treatment. One surgical option is acetabular revision, which is a type of total hip replacement.

The operation involves opening up the hip to expose the acetabular. The size of the skeletal frame must then be assessed and an appropriately sized acetabular cup selected. This cup is secured into the acetabulum, forming a prosthetic hip joint. The surgeon must check that the cup is securely in place by performing a fixation test and a stability test.

When the surgeon is confident that the hip is stable in all directions, the soft tissues can be repaired and the wound sutured together.

Failed acetabular revision surgery

If the acetabular cup is not properly fixed, the surgery will almost certainly fail. This can have serious consequences, as a loose joint can cause the hip joint to dislocate. The operation will then be rendered entirely useless, and the patient will have to undergo another disabling operative procedure.
