GP Referral Delays in Cauda Equina Syndrome CareCauda equina syndrome is, thankfully, a fairly rare condition. However, the severity of its impact and the need for timely and competent medical care should mean that a GP is fully aware of the possible impact of a delay in diagnosis.

If a GP fails to recognise the possible implications of a patient's symptoms or causes a delay in diagnosis, it may be appropriate to make a claim for compensation.

Responding to a patient with symptoms of cauda equina compression

A patient who is experiencing compression of the cauda equina nerves and is at risk of developing cauda equina syndrome may be exhibiting a range of possible signs and symptoms including the following:

  • Lower back pain
  • Pain and altered sensation in one or, especially, both legs
  • Difficulties with ankle reflexes
  • Altered sensation in the saddle area between the legs and around the anus
  • Altered urinary experience such as having to strain to empty the bladder
  • Reduced awareness of needing to urinate
  • Altered awareness of needing to defecate

The GP needs to be aware that such symptoms might indicate compression of the cauda equina nerves in the lower back.

A thorough assessment of the patient is required.

It may also be necessary to refer the patient for an immediate MRI scan.

Where this is not considered necessary, it might be appropriate to advise the patient of the red flag symptoms, such as those above, which should prompt them to attend hospital immediately.

Cauda equina syndrome is a medical emergency and a delay may result in permanent disability for the patient.
