  • A failure to carry out a digital rectal examination leading to a complete failure to diagnose a 3rd or 4th degree tear
  • A failure to recognise and diagnose a severe tear despite carrying out an examination
  • A misdiagnose of a severe tear as a lesser 1st or 2nd degree tear
  • A poor-quality repair which is either deficient in technique or fails to address the entirety of the injury, allowing symptoms to continue and, possibly, worsen
