Third degree tears

A third degree tear is an injury in vaginal childbirth which causes damage to the anal sphincter muscles. Depending on the extent of the injury, the tear is classified in one of three ways:

  • A 3a tear affects less than 50% of the depth of the external anal sphincter (as well as tearing the skin and muscles of the perineum between the vagina and the anus)
  • A 3b tear damages more than 50% of the external anal sphincter
  • A 3c tear additionally damages the internal anal sphincter.

If the injury isn't diagnosed at all, the new mother will receive no repair and her bowel problems will continue and possibly worsen over time. Problems associated with third degree tears include infections of the perineum, incontinence of wind and faeces, discomfort during sexual intercourse, and urgency when needing to go to the toilet.
