In February this year, a Press Association investigation found there had been nearly <\/span>1,200 ‘never events’<\/span><\/a> in the NHS over the last four years.<\/span><\/p>\n

A \u2018never event\u2019 is a serious patient safety incident that can be avoided, as long as the correct guidance and recommendations are implemented. \u00a0<\/span><\/span><\/p>\n

There are currently 14 never events on the list, including wrong-site surgery and retained foreign objects. <\/span><\/p>\n

The Patients Associated described the news as a \u2018disgrace\u2019. But in his blog Learned Friend<\/i>, Nigel Poole QC has questioned whether it would be \u201cmore reassuring\u201d if there were more never events. <\/span><\/p>\n

He draws upon the example of a wrong-site surgery. Currently, wrong-site surgery is a \u2018never event\u2019 if it occurs because the surgeon made a mistake, but not if it occurs due to incorrect laboratory results or referral letters. <\/span><\/p>\n

Mr Poole criticises the \u201ctightly circumscribed\u201d nature of the never event, as it does not matter much to the patient who made the mistake \u2013 the outcome is still the same.<\/span><\/p>\n

He also raises concern that \u201cso few errors are regarded as being wholly preventable\u201d. There are just 14 incidents on the list, meaning that only around 300 mistakes a year are deemed avoidable. <\/span><\/p>\n

Mr Poole suggests this is a very \u201cnarrow view of which errors should never occur\u201d. <\/span><\/p>\n

He writes: \u201cIf systemic barriers were more effective then there would be more errors classified as never events. There would be more mistakes regarded as inexcusable.\u201d<\/span><\/p>\n

\u201cIf systems are improved then the number of errors can be reduced and fewer patients will suffer avoidable harm.\u201d<\/span><\/p>\n
